How to Submit Samples for Analysis

Please read the following guidelines carefully.

IMPORTANT! Microcheck does not accept Biosafety Level 3 or 4 materials or any pathogen that causes highly contagious or otherwise serious disease.

Complete all sections of the Test Request Form

  • DNA Sequencing Identification Test Request Form (Editable PDF)
  • Microscopic Fungal ID Test Request Form (Editable PDF)
  • List sample ID labels clearly.
  • Make a copy of the completed Test Request Form for your records.

Packaging and Shipping Guidelines

  • Seal plates/tubes/slants with parafilm.
  • Enclose samples in Ziploc bag or airtight container.
  • Pack in corrugated box with protective cushioning material.
  • Liquid samples require absorbent material added to packaging.
  • If refrigeration is required, package in an insulating material and include ice packs in Ziploc bags.
  • Enclose the original copy of the completed Test Request Form.
  • Label package "Biological Material for Analysis".
